MCCH Home Care offers a full compliment of home
health care services.
We understand the importance of being home with friends
and family during a period of recovery, and that is why
we work so hard to help our patients receive the care
they need in the environment that suits them
Our goal is to get the patient back on his or her feet
in the least amount of time possible and our Registered
Nurses will design a comprehensive treatment plan in
conjunction with the patient and the attending

for the Home Health Nurse on call. There is a nurse on
call 24 hours a day, 7 days a week for assistance.

Our Registered Nurses provided traditional nursing care
as well as treatment for wounds, dressing changes, IV
set-ups, instructions on medicines and training for
family members who are able to assist.
Home Health Aide/Homemaker
Home Health Aides provide assistance for patients who
are struggling with basic daily tasks such as getting
in and out of bed, walking around, dressing, toileting,
We provide instruction and exercise treatments to help
patients regain strength and independent mobility.
Physical Therapists may also educate patientís families
on techniques for safe lifting and transferring if the
patient needs help when a nurse or aide is not on duty.
Occupational therapists provide patients with new
skills for day-to-day living with physical limitations.
Therapists may also train patients on how to get around
their homes safely with a walking device.
Speech Therapists work with patients who are suffering
from speech loss as a result of disease, surgical
procedure or after suffering from a stroke.
Medical Social Services
Social workers visit the home to assess the patientís
needs and to recommend other community resources that
might prove valuable to the patient and family. They
are able to connect the patients, as well as family
caregivers, with programs that will maximize the
patientís recovery and future wellbeing. Social workers
may also provide basic counseling to help patients and
family members adjust to the demands of at home care.
