Membership Freezes
Members may freeze the monthly membership dues for reasons such as: illness, vacation or extreme work conditions. The Center must receive written notice in advance of the freeze whenever possible. Membership freezes will be for a minimum of one (1) and maximum of three (3) months. Memberships may not be held for half months or weeks. Members may place their membership on hold up to two (2) times each year. Members will be responsible for any fees assigned by their financial institution related to Electronic Fund Transfers. There will be a $7.00 per person freeze fee per month.
Membership Dues
All memberships have the privilege to use the Center for Health & Wellness facilities upon payment of dues and completion of the membership application. Membership dues may be paid by electronic transaction, credit card, check, or cash.
The Center for Health & Wellness shall determine the amount and terms of payment of dues payable by members on a monthly basis. The obligation to pay dues is not dependent upon availability of the facility. The member should understand that the amount and extent to which he/she participates in programs is his/her own responsibility. Failure to utilize the facility does not excuse the member from the obligation to pay the monthly dues nor constitute grounds for a refund.
Prepayment
Members choosing to pay for 12 months in advance will receive a 10% off the standard monthly membership fee. Prepaid dues are non-refundable, except in the case where the member is unable to participate due to changing health status or relocation out of the geographic region.
Returned Checks
A service fee of $25 per item will be assessed for a returned check, credit card draft or electronic fund transfer as a result of insufficient funds, closed account or similar circumstances.
Delinquent Accounts
Members failing to make payment within 30 days of payment due date will be assessed a 5% late fee.
Cancellations
You may cancel your membership any time within the first 10 days of joining in order to receive a full refund. No refund will be provided after the orientation appointment. If you need to cancel your membership after the first 10 days have passed you may do so with a written 30-day notice to the Center or completion of the cancellation form. Membership cards must be returned to the Center at the time of cancellation to avoid a $5.00 card fee. Please note that some memberships require a 6 or 12 month contract. Cancellation before the end of the contract period will require full payment of the contract.
Appointment “No Show” / Late Cancellation Policy
Giving our undivided attention to each and every customer is important to us, as we’re sure it is to you. We block out the time for your visit accordingly. There is a $25 no-show/late-cancellation fee for any missed appointment at the Center for Health and Wellness.
