Steps to Enroll

Click on the MyHealth patient portal Link
Complete self enrollment form. Fill out your name, date of birth, e-mail, and last four digits of your social security number.
Access your e-mail for your one-time user name, password, and link to the Portal.
Click the link to access the portal
Enter your one-time user name, password, and security question and click Log On
Enter your new user name and password
